We offer residency trainees an collaborative, interdisciplinary team learning environment, exceptional community based care and one-on-one mentorship with leaders in Family Medicine. We strive to provide a peaceful environment for our patients to receive high quality, compassionate and holistic health care as well as be leaders in our community. The "Wise Elephant Family Health Team” is located in the heart of downtown Brampton. It operates out of 2 renovated Victorian homes that stand as an established fixture within the community.
Currently, our Family Health Team is composed of 8 physicians (of which Drs Banerjee, Goel will be your primary preceptors) and is supported by 2 nurse practitioners, a Mental Health worker, a palliative care nurse, and an occupational therapist. Our physicians have varied backgrounds that contribute to a comprehensive approach to medicine. Specializations include low-risk obstetrics, palliative care, rehabilitation medicine and complex continuing care, health administration, research, global health, GP psychotherapy and emergency medicine. We have links with Peel Public Health, the Central West LHIN, and one of our members is a Peel Region Coroner. We believe in working in a collaborative, multidisciplinary environment and believe in a culture of continuous quality improvement and innovation from all of our physicians, interdisciplinary health practitioners and staff.
Since our FHT's inception in September 2010, we have developed programs for chronic disease management, palliative care, mental health and addictions, care of the elderly, smoking cessation and a well woman/maternal newborn program.
In congruence with our value on patient centred care, we have launched a Patient Health Portal called miDash (www.miDash.ca) that allows our patients to access their real time health information. We currently are involved in the Imagination Nation Challenge run by Canada Infoway.
Our Family Health Team is also involved in an international health project in a rural village in the Foothills of the Himalayas. For the past 3 years we have taken family medicine residents to do an international health elective with us. You may find more information on this at www.IndiaProject.ca.
Our Family Health Team's vision is to the be "the best FHT in Ontario!". To achieve this goal, we believe training is the next generation of leaders in family medicine is imperative.
